Registration rates are detailed on the conference website. If you have any queries about the pass type for which you are eligible, please email our office at abnregistration@affinityevents.co.uk before completing this form.

Invoice address. The details entered here will appear on your invoice / receipt for the meeting.
 

Pass type

Member pass types: These rates are for current members only and should be accompanied by your membership number. 
If you incorrectly register as a member, your pass type and invoice will be changed to that of a non member.

Invited Speaker: This category does not include Special Interest Group (SIG) sessions which are planned and managed independently from the main conference.

Student pass: This rate is for medical undergraduates only. 
If you book this rate in error and are not eligible for the student rate, your pass type and invoice will be changed to that of a non member.
